Step 1: Click Complete via DocuSign:


Step 2: Fill out your name and email, then click Begin Signing


Step 3: Read the Electronic Record and Signature Disclosure, then click I agree to use electronic records and signatures. Then Continue:


Step 4: Complete all required fields then click Next:


Step 5: Review and Click Finish:


Step 6: Download a copy for your records and then click Close:


Step 7: Click Check on Status

Once you have successfully completed the DocuSign, there will be two attachments included in response to this question. One attachment will be your electronic signature on the forms provided by the agency, the other will be a summary of the completed signature (receipt and time logs):
